Appointment Policies
24-Hour Cancellation & Rescheduling Policy
All cancelations or changes within 24-hours of the appointment will be charged in full.
Payment
Payment is due at the time of your appointment, I accept cash, most credit/debit cards, & Venmo.
Insurance
Currently not in-network however, we are happy to bill your insurance company for you or provide you with a receipt.
Your First Appointment
Please arrive 15 minutes early to fill out paperwork or you can fill out the form below and bring it with you to your first appointment.
What to Bring
Please bring with you any relevant copies of lab work or test results, and a list of any medications or supplements that you are currently taking.
Feel free to wear comfortable, loose clothing — but it’s not necessary. We’ll supply everything you need!
It’s best to show up to your appointment neither too hungry, nor too full. Optimally eating about an hour before your appointment.
